Welcome to Meadows Chiropractic!
Getting to know you and your needs begins before your first visit to our practice!
When you schedule your appointment, you will receive an email from us. That email contains all the important information for your 1st visit, including appointment time, office location and a link to your new patient paperwork/health questionnaire. This questionnaire needs to be completed before the day of your appointment.
There is a $60 deposit required to book an appointment that will go towards your 1st visit cost (this is not the cost of the appointment). This deposit will also cover a late cancellation fee in the case of a late cancel (24 hours) or no show.
