There will be a late cancellation/reschedule fee for appointment cancellations or reschedules less than 24 hours before your appointment, missed appointments, or late arrivals that need to be rescheduled.
All massage appointments require a 48-hour cancellation.
If you will be more than 15 minutes late, please call our office, you may have to be rescheduled.
Late fee *maybe* waived if your appointment is rescheduled BEFORE your appointment time for the same day or same week. Late fees for massage appointments cannot be waived.
Cancellation Fees - $35 PER SERVICE scheduled (NOT PER APPOINTMENT). $55 fee for 90-minute massages
Saturday Cancellation Fee – FULL APPOINTMENT AMOUNT
Late fees will be charged immediately to the credit card we have on file. Insurance will not pay, nor can office credit be used. You are welcome to give us a different card by phone when you call.
A missed appointment or late arrivals due to no reminder will still result in a fee. We love technology, but we have no control (or even knowledge when) it temporarily malfunctions. You are responsible for knowing what day and time you scheduled your appointment. Please call if you have forgotten, have not gotten a reminder, or have received an incorrect reminder.
Returned Check Fee - $30
If your account is turned over to our collection agency, you will be responsible for the collection fee charged to us by the agency in addition to your outstanding balance.
All copays, deductibles and co-insurance are due when services are rendered and are based on what your insurance verification states on the date of service.
We are not responsible for incorrect or incomplete information received by you or your insurance company. You are responsible for updating us with new insurance information if it changes for any reason.
Health insurance coverage is a contract between you and your insurance company. You are responsible for knowing your insurance benefit information, i.e., deductibles and visit limits.
